Instructions

Our application form has three sections: One requires completion by you, one by a Christian friend that has known you for over two years and one by your pastor. Click the blue download button below and when it opens, complete the required fields and print the form. Go through the checklist and ensure everything is completed. Sign the completed form and mail it (or deliver it) with all associated fees to the address below.
